CHS staff members are health and human service professionals who are available to assess your circumstances and discuss your needs in order to help facilitate connection to appropriate resources and care. The service menu of CHS includes:
- Information, education and support with Access to Health Care
- Case Management support to identify community resources
- General assistance evaluation for residents of Derry, Hampstead and Londonderry.
- Opportunities to increase knowledge and skills which may enhance your ability to independently navigate the health and human services system.
If you are struggling to navigate the complicated system of service access and need help with that, CHS is ready to assist. While appointments are recommended it is almost always possible to speak with a member of the staff prior to coming in to discuss your individual circumstance prior to a meeting.
CHS is open Monday through Friday 8:30am to 4:00pm.
